MULTISYSTEM:
All the data is given in the right order the chief complaint, the history of present illness, the personal history was given in detail, the information regarding the treatment was regularly updated all the lab reports were provided.

CNS:
The history of the patient was given in a systematic manner the diagnosis and treatment used for the patient was mentioned, the advice at discharge was provided all the lab reports and updates on the case were provided.

RENAL:
All the data of the patient is in the correct order and correctly arranged. All the investigations are done perfectly. All investigations (and their dates) were logged in.

CVS:
The history of present and past illness is coherent and well established. The videos are great for understanding the case. The case sheet has captured all the relevant data in the right order. Correct terminology was used. The reports of all the lab investigations conducted were given.

ABDOMINAL:
The history of the patient is coherent and brief. The diagnosis and the treatment of the patient are given properly and to the point. The reports of all the lab investigations conducted were shared.

-Inj.LASIX 40mg is given to treat the symptoms of fluid retention like edema.
-IVF_Ns@UO+50ml/hr.
-inj.MAGNEX FORTE 1.5gm: it is used to treatment of severe or serious bacterial infections like urinary tract infections.
-Tab.NODOSIS XT :it is used in kidney disease ,mainly acts as antacid .
-inj HAI
-nebplain Asthalin 4 results.
-tab.OROFEA XT:it is a dietary supplement containing ,combination of folic acid and iron.
-tab ULTRACET :it acts as pain relieving medicine.

*the investigations and drugs given to the patient is satisfactory .where the drugs are very well described according to their action.The investigations made easy to diagnose the case .

A CASE OF LIVER ABSCESS IN RIGHT LOBE

August 01, 2021 CHIEF COMPLAINTS Fever since 1 week Dyspnea since 1 week Burning micturition since 1 week  Pain abdomen since 1 week HISTORY OF PRESENTING ILLNESS A 45 year man, was apparently asymptomatic 14 years back.  14 years back  during his wife's  pregnancy on routine investigations she was found to be RVD positive. On getting tested, he turned out to be RVD positive as well.  He was started on medications which he takes regularly ( not documented ), he says he takes 2 tablets at night. And his wife takes one tablet at night. On asking whether their child was also RVD positive, his wife told us that their child's test came out to be negative.
